This patient presented 2 days ago with a lump in his eye.
He is a retired GP, with a history of pterygium.
Spot diagnosis: this is a HUGE ocular surface neoplasia.
Such neoplasia CAN develop at the head of pterygium.
Whilst there is a chance it is non-invasive, the risk here is ocular invasion and perineural spread.
Lesson: with good regular assessments by optometrists, most malignant transformations of pterygium can be identified early and treated.
Lesson: any clinician can trip over their own diagnosis!
